For most students, nurses and interns, the Littmann Classic III is the one to buy: a dual-sided, tunable stethoscope with a 5-year warranty at a mid-range price. The Cardiology IV is the step up for doctors in emergency, ICU and cardiology work, with a larger chestpiece, a deeper bell, dual-lumen tubing and a 7-year warranty.
The short answer
- Classic III: tunable diaphragms on both sides, single-lumen tubing, 5-year warranty, mid-range price.
- Cardiology IV: chestpiece 40% larger and bell 60% deeper than the Classic III, dual-lumen tubing, 7-year warranty, premium price.
- Both are made in the USA, use the same spare parts kit, and can be laser-engraved.
- Buy the Classic III for medical school and general wards; the Cardiology IV once you know you are heading into acute or cardiac work.
What the Classic III and Cardiology IV share
It helps to start with what you do not give up by choosing the Classic III. Both models have:
- A dual-sided stainless steel chestpiece with tunable diaphragms on the adult and paediatric sides. Light pressure brings out low-frequency sounds; firmer pressure brings out high-frequency sounds, without turning the chestpiece over.
- A paediatric side that converts to a traditional open bell, with a non-chill bell sleeve.
- Soft-sealing eartips for comfort and a good acoustic seal.
- A 69 cm tube made without natural rubber latex.
- The same diaphragm and eartip spares: the Littmann 40016 spare parts kit fits both.
So the Classic III is not a stripped-down Cardiology. It is the same tunable design in a smaller, lighter-built package.
What is different
Chestpiece size and bell depth
Littmann makes the Cardiology IV chestpiece 40% larger than the Classic III's, with a bell 60% deeper. A bigger diaphragm and deeper bell are built for picking up hard-to-hear sounds: aortic stenosis, S3 gallops and faint pulmonary sounds. That is why the Cardiology IV is aimed at the emergency department, ICU, cardiac ICU and step-down units.
Tubing
The Classic III uses single-lumen tubing: one sound path in one tube. The Cardiology IV is a dual-lumen design: two sound paths inside one tube, so you avoid the rubbing noise of twin external tubes. It is also why cases shaped for single-lumen Classic models do not fit Cardiology models.
Warranty and price
The Classic III carries a 5-year Littmann warranty and the Cardiology IV carries 7 years. As an authorised Littmann distributor, our stock is covered by the full Littmann warranty. The Classic III sits in the mid-range; the Cardiology IV is a premium stethoscope and costs roughly twice as much.

Our verdict
Choose the Classic III for medical school, nursing and general wards. Choose the Cardiology IV when faint sounds in noisy, acute settings are your daily work.
Which Littmann stethoscope is best for doctors?
It depends on where you work, not on your title. A Classic III does the job in general wards, clinics and general practice, and it is listed for interns and GPs as well as students. If you are heading into emergency medicine, anaesthetics, ICU, internal medicine or cardiology, the Cardiology IV is worth the extra: the larger chestpiece, deeper bell and dual-lumen tubing are built for faint sounds in noisy settings, and the 7-year warranty covers you through registrar years.
One way to think about it: a Classic III covers you through medical school, and a Cardiology IV makes most sense once you start internship or community service and know which way you are heading. If you already know you are going into acute or cardiac work and would rather buy once, start with the Cardiology IV.

Our verdict
For small patients, the Classic II Paediatric (3.3 cm diaphragm) and Classic II Infant (2.7 cm) sit alongside these, each with a 3-year warranty.
Littmann Lightweight vs Classic III
The Lightweight II S.E. is Littmann's entry-level adult stethoscope and, at 118 g, the lightest. Its teardrop chestpiece slides easily under a blood pressure cuff and around bandages. It has a tunable diaphragm on one side and an open bell on the other, with a 2-year warranty. The Classic III costs more but gives you a tunable diaphragm on both sides and a 5-year warranty.
For medical, physio and paramedic students who will auscultate hearts and chests on every rotation, the Classic III is the better long-term buy.
Littmann Classic II vs Classic III
The adult Classic II S.E. has a tunable diaphragm on the adult side and a conventional bell on the other. The Classic III has tunable diaphragms on both sides, and its paediatric side converts to an open bell, so you can use the small side on children, thin patients and carotids. We no longer sell the adult Classic II S.E., but we still stock its spare parts kit. The Classic II name lives on in the Paediatric and Infant models.
Littmann CORE Digital vs Cardiology IV
The CORE Digital is Littmann's digital stethoscope. It amplifies sound, reduces background noise and can record through an app. It is not on our shelves at the moment, so contact us if you need one. For most clinicians an acoustic Cardiology IV is the simpler buy: there is nothing to charge or pair.
Colours, finishes and Satin tubing
Within a model, colourways share the same chestpiece design and warranty. You are choosing a look. The standard stainless steel versions, like the Classic III in burgundy, are the most affordable. Mirror, rainbow, smoke and champagne finishes, like the Classic III with a navy tube and mirror finish or the Cardiology IV burgundy champagne finish, cost a little more.
Littmann's Satin Finish tubing has a micro-textured coating that resists cling, stains and scratches and feels softer around the neck. Only the tube changes; the acoustics are identical. See the Classic III Satin Finish in black.

Engraving, spares and delivery
We laser-engrave a name on the chestpiece of any Littmann stethoscope, usually on the same day. Our engraving guide shows examples. A Littmann ID tag fits every Littmann model and is another way to stop yours going missing on a ward round.
When the diaphragm or eartips wear out, you do not need a new stethoscope. The black 40016 or grey 40017 spare parts kit replaces the adult and paediatric diaphragms and eartips on a Classic III or Cardiology IV.

Questions people ask
Is the Cardiology IV worth it over the Classic III for a medical student?
For most students, no. The Classic III has tunable diaphragms on both sides, a 5-year warranty and costs about half as much. The Cardiology IV makes sense once you know you are heading into emergency, ICU, internal medicine or cardiology, where its larger chestpiece, deeper bell and 7-year warranty earn their price.
Do the Classic III and Cardiology IV use the same spare parts?
Yes. The Littmann 40016 (black) and 40017 (grey) spare parts kits fit both. Each kit has adult and paediatric tunable diaphragms and two small eartips.
Does Satin Finish tubing change how a Littmann sounds?
No. Satin Finish changes only the tube, which gets a micro-textured coating that resists cling, stains and scratches. The chestpiece, diaphragms and acoustics are the same as the standard model.
Which Littmann stethoscope is best for nurses?
For blood pressures and routine observations, the Lightweight II S.E. is light at 118 g and its teardrop chestpiece fits under a BP cuff. Nurses who auscultate chests regularly often prefer the Classic III for its tunable paediatric side and 5-year warranty.
Can you engrave my name on a Littmann stethoscope?
Yes. We laser-engrave a name on the chestpiece of any Littmann stethoscope, usually on the same day.
